Payment Plan Options Our office provides several convenient payment options, none of which impose interest or finance charges. We make an effort to accommodate various benefit plans and if available your flexible spending account or cafeteria plan. For those with dental insurance (with an orthodontic benefit available), in most cases our office will send in an insurance claim for pre-approval, prior to arranging a payment plan with you. Insurance benefits available will be subtracted from the fee and the remaining balance financed directly with you. Please note that you are fully responsible for the entire fee charged should insurance change, terminate or if we have difficulty collecting for any other reason. We make our best effort to handle all insurance matters, although we may request your assistance if we have difficulty collecting your benefits. Please note that utilization of a credit card or direct withdrawal option in which there are insufficient funds, returned checks, or maxed-out limits, a fee may be assessed to cover normal bank service charges. Unless otherwise provided for in your financial contract with us, payments must be current prior to removal of the orthodontic appliances.
OPTION 1 - Down payment of 1/3 of the total fee due at the beginning of treatment with monthly or quarterly payments thereafter. OPTION 2- Equal monthly payment using direct withdrawal from a checking / savings account (please provided a voided check if you are using your checking account or savings deposit slip for a savings account withdrawal) OPTION 3- Equal monthly payment using your Visa, MasterCard, American Express or Discover Card. You will be responsible for providing up to date, current card information. OPTION 4- Use of flexible benefits (pre-tax dollars) on a monthly or quarterly basis. Generally monthly reimbursement is less hassle and most commonly used for those wishing to maximize pre-tax dollars and minimize time spent getting reimbursed. OPTION 5- Pre-payment of the entire (uninsured portion) of the fee will result in a 5% discount.
